The national information standard, which specifies the core record content to support the provision of high-quality co-ordinated care at the end of life, was published 20 March 2012 and last revised in September 2015.

This record keeping guidance, developed in consultation with health and social care professionals, supports staff using electronic or other end of life care co-ordination systems. It should be used in conjunction with the extensive guidance available to support clinical practice, advance decision making and end of life care service provision.

The guidance has been endorsed by:
