National information standard

When a person is approaching the end of life, all professionals involved in their care should have secure access to complete and up-to-date information regarding their expressed preferences. It is a case of the right person, having the right information, at the right time, which can make all the difference to the quality and experience of care for an individual, their family and carers.

The national information standard, Palliative Care Co-ordination: core content (SCCI1580), specifies the key information that is needed for high-quality co-ordinated care, and provides structures and definitions to support effective and secure recording of this data.

It is a core data set that enables consistent, accurate recording of the relevant facts for each person who is receiving palliative and end of life care.

The information standard is mainly intended for use with Electronic Palliative Care Co-ordination Systems (EPaCCS), and is required in these situations. 

It was developed following research and consultation with NHS and social care staff. The standard has been approved by professional organisations representing doctors and nurses, along with allied health and social care professionals.
